STEP #1:
COMPLETE AND SUBMIT FITNESS CHALLENGE ONLINE REGISTRATION FORM
By submitting this registration form, I have read and understand
the terms and conditions, health statement, waiver and release of liability in full. I understand that I am voluntarily
participating in the Celbod Fitness Challenge.
Terms &
Conditions:
All contestants
must be 18 years of age or older. Youth are permitted to register
with parent or guardian written consent.
Registration Deadline Has Been Extended Until
Sunday, September 20th
All registered contestants
must wear work-out apparel (shorts or leggings and tank top) at
weigh-in.
All registered contestants
must schedule a mandatory before contest weigh-in (weight, bodyfat,
and measurements) to be completed by
September 20, 2008.
All registered contestants
must schedule a mandatory after contest weigh-in (weight, bodyfat,
and measurements) to be completed by
November 9, 2008.
Your registration fee covers the following
services: before and after measurements and a fitness profile
summary.

Health
Statement:
Successful weight loss and healthy weight management depend on
sensible goals and expectations. If you set sensible goals for
yourself, chances are you'll be more likely to meet them and have
a better chance of keeping the weight off. In fact, losing even
five to ten percent of your weight is the kind of goal that can
help improve your health.
a.
Most overweight people should lose weight gradually. For safe and
healthy weight loss, try not to exceed a rate of two pounds per
week. Sometimes, people with serious health problems associated
with obesity may have legitimate reasons for losing weight
rapidly. If so, a physician's supervision is required.
b.
What you weigh is the result of several factors: how much and what
kinds of food you eat, whether your lifestyle includes regular
physical activity, whether you use food to respond to stress and
other situations in your life, your physiologic and genetic
make-up, your age and health status.
Successful weight loss and weight management should address all of
these factors. And that's the reason to ignore products and
programs that promise quick and easy results, or that promise
permanent results without permanent changes in your lifestyle
c.
Check with your doctor. Make sure that your health status allows
lowering your caloric intake and increasing your physical
activity.
d.
Follow a calorie-reduced, but balanced diet that provides for as
little as one or two pounds of weight loss a week. Be sure to
include fresh fruits and vegetables, along with whole grains, lean
meat and low fat dairy products.
Waiver &
Release of Liability:
By
participating in the contest, contestants agree to have their
name, voice, or likeness used in any advertising or broadcasting
material relating to this contest without additional financial or
other compensation, and, where legal, to sign a publicity release
confirming such consent prior to consent prior to start of
contest.
I agree to hold Celestial Bodies Inc (Celbod), its corporate
licensee, its parent, subsidiary and affiliated corporations, and
the officers, shareholders, directors, employees, agents and
representatives of each of them harmless against any and all
claims or liability arising directly or indirectly from the prize
or participating in the Fitness Challenge contest.
Online
Training Package: $75.00 Per Person
Contents include: nutrition
guide, meal plan, exercise program, fitness logs and customized
profile summary. Also included is access to private fitness blog
site and
support group meetings.
The Online Training Packagte does not include personal training
session
Registration
